COVID Vaccine Recorder - NYS Excelsior Pass

To register for a New York State Excelsior Pass

1. Go to https://epass.ny.gov/home and choose Get Started

2. Accept the Authorization to Disclose
3. On the Let’s Get Started page enter the information requested
   

4. Next, enter your vaccine information

5. After selecting Verify, you will get your pass

Upload your pass to the COVID Vaccine Recorder

To upload your pass to the SUNY Cortland COVID Vaccine Recorder there are two options

Print the pass

1. Print your pass using the command using the command at the bottom of the page with the pass on it

2. Use the directions for Taking a Photo or Scanning to capture an image of the pass

Or

Take a screenshot

1. While on the webpage with your pass open, select the Windows Start menu
2. Choose the Snipping Tool (it is found in the Windows Accessories folder)

3. When the Snipping Tool opens choose New

4. Your cursor will change to a plus sign, draw a box around the image of the pass by starting at the top left corner and drawing a diagonal to the bottom right corner of the image you would like to capture.
5. Select the Save icon

6. Your File Manager will open. Choose where you would like to save the image.
7. Open the SUNY Cortland COVID Vaccine Recorder and enter the necessary information
8. Select the Attach file button and retrieve the screenshot from the location where you saved it.
