General Formstack Account Terms of Use
- Agree to SUNY Cortland Data Classification Policy terms
- Agree to SUNY Cortland Cyber Security Standards
- Agree to SUNY Cortland IR Acceptable Use Policy
- Agree to SUNY Cortland IR Data Confidentiality P&P
- Agree to SUNY Cortland Personally Identifiable Information Policy
Inactive Account Reclamation
- Any account that has not had a login within the last 180 days will be considered inactive.
- You will receive a notice around 30 days and around 14 days prior to account removal.
- Any form that you created will still exist after your account has been reclaimed.
- After your account has been reclaimed, should you want an account again, you will need to submit a new account request and go through the approval process again.
- If you will be out for over 180 days and want to reserve your account, please contact PI by clicking on the Online Forms Support button in the upper right of this page.
Form Archiving Procedure
- Forms created two or more years ago that have also not received submissions over the past two years are subject to periodic archiving.
- Contact PI to submit a form for white-list consideration.
- All Formstack users will receive a list of their archived forms.
  - Please contact PI by clicking on the Online Forms Support button in the upper right of this page within 30 days if you need a form recovered.
