Chosen Name Custom Email Instructions
Summary
How to set up notification emails in Formstack that uses a responder's preferred name instead of legal name if one is provided.
Body
- Open Formstack in your web browser of choice (we recommend Firefox).
- Open the form whose emails you would like to customize.
- Ensure that the following fields are included on the form:
- Name
- “Do you have a different chosen name?” or similar phrasing
- Use the Radio Button field with “Yes” and “No” options
- Mark as Required
- Chosen Name
- Add logic to Chosen Name
- Show this field when all of the following rules match: “Do you have a different Chosen name?” is Yes
- Click on Settings on the right-side of the form’s navigation bar
- Click on Emails & Actions on the left-side of the settings bar
- Add a Notification Email
- Adjust the Email Name to include “ – chosen”
- For “Contents”, select “Custom Message”
- Type out your desired message, leaving a space where you would like the name to be included.
- Click and drag the Chosen Name form field (or the First/Last Name fields below it if you do not wish to use their whole name) into the space you left in the message.
- Verify the remaining options in the email are to your liking, and then click Save Notification Email
- Repeat steps 7-12 for the legal name (the field titled “Name”)
- You should now have two notification emails- one for legal names and one for chosen names. On the right-hand side of the emails, you will see “Logic”. Click it.
- Select “Add Field Logic Rule” and then set the following logic, being sure to click Save after each:
- Chosen Name
- Send email when all of the following Field Logic rules match:
Do you have a different chosen name? is Yes
- Legal Name
- Send email when all of the following Field Logic rules match:
Do you have a different chosen name? is No
